The British Columbia Ambulance Service (Canada) has the largest coverage area.

364,000 square miles covered by 500 ground ambulances, 4 helicopters, and 6 planes, operating out of 190 stations. BCAS has 3500 Paramedics serving a population of 4.4 million and respond to over 500,000 calls per year. BC Ambulance also boasts the world's only Paramedic based Neonatal-Intensive Care/Infant Transport Team.

BCAS is big, there are a few others that are bigger down under as well

St john New Zealand is country wide, mostly volunteers, but just under 3000 paramedics and in Australia ambulance NSW has a bit over 4000 parameidcs, 5 helicopters as well as contracts and other fixed wings, ambulance victoria has a bit over 3000 with 4 helicopters and thier own fixed wings :) no idea about callouts but the services are stretched so id guess alot
